New Patient Registration

The Practice is pleased to accept patients from Trowbridge, Holt, Semington, Keevil, North Bradley, Southwick, Hilperton, Wingfield and Westwood and continues to grow successfully.

Ours practice area is the BA14 postcode

New Patients will need to provide photographic ID.

If registering a child under 16 we will need to see a birth certificate.

If providing this information is a problem please contact reception for advice.

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire. 

Medical treatment is available 5 working days from the date of registration.  Please contact reception for further information.

If you have regular medication on a repeat prescription you will need to arrange a telephone consultation with one of our GP's to set this up prior to ordering from us for the first time.(it will not automatically transfer from your previous practice) 

Your Registration application will also include registration for our on line services

When you have completed all of the details, please print  your form bring to the surgery along with appropriate ID. Once all the completed paperwork is handed in at Reception, we ask for five working days to complete the registration before any appointments can be made. Please ask at reception if you require information about opting out of Data sharing from your records.

When you have completed the pre- registration form you will be sent an online password via text.

To complete your registration please log into your account and submit the New Patient health Questionnaire 

You will also need to supply ID.Once this has been seen you will be given full online access and will be able to request medication and book appointments online .
