New Patient Registration

The Practice is pleased to accept patients from Trowbridge, Holt, Semington, Keevil, North Bradley, Southwick, Hilperton, Wingfield and Westwood and continues to grow successfully.

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire. 

Medical treatment is available 5 working days from the date of registration.  Please contact reception for further information.

If you have regular medication on a repeat prescription you will need to arrange a telephone consultation with one of our GP's to set this up prior to ordering from us for the first time.(it will not automatically transfer from your previous practice) 

Your Registration application will also include registration for our on line services

When you have completed the pre- registration form you will be sent an online password via text.

To complete your registration please log into your account and submit the New Patient health Questionnaire, you will be given online access and will be able to request medication, book appointments and check your records (from April 2020) online.

Please  click on the link below to open the registration form. When you have completed all of the details, please print  your form & post it to us or bring to the surgery (Please note that during the covid 19 pandemic you will need to post your form in the letterbox). 

Once all the completed paperwork is received , we ask for five working days to complete the registration before any appointments can be made. Please contact reception if you require information about opting out of Data sharing from your records.
